ENT Consultants of East TN has three offices located within teh Knoxville TN area.   The 15 provider group is interested in replacing there current Medical Manager product with a new Practice Management, EHR and Patient Portal integrated application.  This Request for information has been sent to 15 potential vendors with a goal of selection 4 of teh top vendors for on-site presentaions in November of early December 2017 with a goal of signing a contract before December 31, 2017.

To assist potential vendors, I have attached a number of documents that helps describe the practices workflows and current paper forms that are used.  Each vendor has approval to download the files in order to get a better understanding of the practice's needs.

The ENT Practice also has a small Allergy Practice with about 350 active patients.  Today everything is on Paper including all shot records.  The Practice is looking for innovative approaches to capturing shot records without increasing staff time. 

The ENT Practice has a large Audiogy practice with 5 locations.   Ecah location is using a GSI https://www.grason-stadler.com/solutions device for capturing hearing information with 15,000 patients with 9,000 new patients since 2013.   Below are a number of paper forms they currently use.
